DBT - The Dialectical Dance
Developed by Marsha Linehan, DBT is a cornerstone therapy for Borderline Personality Disorder (BPD), rooted in the biosocial theory of emotional dysregulation. It addresses the interplay between biological vulnerability and an invalidating environment. The core dialectic is balancing acceptance and change, helping patients learn to manage intense emotions and reduce self-harm behaviors like cutting.

DBT integrates individual therapy, group skills training, and phone coaching. The four core skills modules are taught in group sessions:
| Module | Goal | Key Skills / Mnemonics (📌) |
|---|---|---|
| Mindfulness | ↑ Awareness of the present moment | "What" skills (Observe, Describe, Participate) "How" skills (Non-judgmentally, One-mindfully, Effectively) |
| Distress Tolerance | Survive crises without worsening them | ACCEPTS: Activities, Contributing, Comparisons, Emotions, Pushing away, Thoughts, Sensations TIPP: Temperature, Intense exercise, Paced breathing, Paired muscle relaxation |
| Emotion Regulation | Understand & ↓ vulnerability to emotions | Check the Facts, Opposite Action, Problem Solving |
| Interpersonal Effectiveness | Maintain relationships & self-respect | DEAR MAN: Describe, Express, Assert, Reinforce, Mindful, Appear confident, Negotiate |
MBT - Mind-Reading Mastery
- Core Idea: A psychodynamic therapy, developed by Peter Fonagy & Anthony Bateman, centered on enhancing mentalization-the ability to understand behavior in oneself and others in terms of underlying mental states (thoughts, feelings, intentions).
- Mechanism: Improves reflective functioning (RF), the operational measure of mentalization. It helps patients move from a state of "psychic equivalence" (internal world = external reality) to a more reflective, curious stance.
- Therapeutic Stance & Techniques:
- 'Not Knowing' Stance: Therapist is inquisitive and avoids assuming they know the patient's internal state.
- Attachment Theory: Views BPD as a disorganization of attachment, where trauma impairs the capacity to mentalize.
- Affect Focus: Explores emotions to understand the mental states behind them.

⭐ Exam Favorite: While used for various personality disorders, MBT was specifically developed for and is a first-line evidence-based treatment for Borderline Personality Disorder (BPD).
📌 Mnemonic: M-B-T → Mind Behind the acT
- Dialectical Behavior Therapy (DBT) is the first-line treatment for Borderline Personality Disorder (BPD), particularly for managing self-harm and suicidality.
- DBT focuses on four key skills modules: mindfulness, distress tolerance, emotion regulation, and interpersonal effectiveness.
- Mentalization-Based Therapy (MBT) is also effective for BPD, aiming to improve the ability to mentalize-understanding the mental states of oneself and others.
- MBT helps patients link thoughts and feelings to actions, improving affect regulation.
