Blood Flow Pathway - The Double-Loop Journey
- Pulmonary Circuit (Low-Pressure): The right side of the heart pumps deoxygenated blood through the pulmonary arteries to the lungs for gas exchange. Oxygenated blood then returns to the left atrium.
- Systemic Circuit (High-Pressure): The left side of the heart pumps this oxygenated blood via the aorta to the rest of the body. Veins and vena cavae return deoxygenated blood to the right atrium.
⭐ Physiological Shunt: A small volume of deoxygenated blood from the bronchial circulation (supplying lung tissue) and Thebesian veins (draining the heart muscle) empties directly into the left atrium. This is a normal finding.
Coronary Circulation - The Heart's Fuel Line

- Arterial Supply: Originates from the root of the aorta; perfused during diastole.
- Left Coronary Artery (LCA) Branches:
- Left Anterior Descending (LAD): Supplies anterior 2/3 of IV septum, apex, anterior LV wall. The "widow maker."
- Left Circumflex (LCX): Supplies lateral/posterior LV wall, anterolateral papillary muscle.
- Right Coronary Artery (RCA): Supplies RA, RV, SA & AV nodes.
⭐ Coronary Dominance: Determined by which artery gives rise to the Posterior Descending Artery (PDA). Right dominance is most common (~85%). Occlusion of the RCA can thus cause AV nodal block.
Cardiac Conduction - The Heart's Electrical Beat
- Sinoatrial (SA) node: The heart's primary pacemaker, firing at 60-100 bpm.
- Atrioventricular (AV) node: Delays the signal to allow for ventricular filling; intrinsic rate of 40-60 bpm.
- Bundle of His & Purkinje fibers: Transmit impulse to the ventricles; intrinsic rate of 20-40 bpm.

⭐ The AV node's refractory period is crucial; it protects the ventricles from dangerously high rates during atrial tachyarrhythmias like atrial fibrillation.
