• Primary HPT Labs: ↑ PTH, ↑ Serum Ca2+, ↓ Serum Phosphate, ↑ Alkaline Phosphatase.
• Secondary HPT: Usually from chronic kidney disease; ↑ PTH, but Ca2+ is low/normal.
• Hypoparathyroidism: Often post-surgical; ↓ PTH, ↓ Ca2+ (tetany), ↑ Phosphate.
• Pseudohypoparathyroidism: PTH resistance (GNAS1); ↑ PTH, ↓ Ca2+; linked to Albright's osteodystrophy.
• MEN syndromes (1 & 2A) frequently feature parathyroid hyperplasia/adenomas.
