- **OTC deficiency**: ↑ NH3 and **high** orotic acid (excess carbamoyl phosphate is shunted into the pyrimidine synthesis pathway).
- **UMP synthase deficiency**: **High** orotic acid, but **normal NH3** (it is part of pyrimidine synthesis, not the urea cycle).
- **HGPRT deficiency**: High uric acid (Lesch-Nyhan syndrome); does not primarily affect NH3 or orotic acid levels.
