Sepsis is one of those "all hands on deck" emergencies where every minute truly counts. For the UKMLA AKT, you'll need to be sharp on the immediate management steps and how we identify these patients quickly on the ward.

The Sepsis 6 Bundle

This is the gold standard for initial management. The goal is to complete all six steps within one hour of identifying sepsis. I like to remember them as "3 In, 3 Out":

3 Out (Take/Monitor):

  1. Blood Cultures: Take these before giving antibiotics if possible, but don't let it delay the meds!
  2. Lactate: Check a blood gas to see how much tissue hypoperfusion is happening.
  3. Urine Output: Insert a catheter to monitor fluid balance and renal perfusion.

3 In (Give):

  1. Oxygen: Aim for saturations of 94-98% (or 88-92% in COPD).
  2. Antibiotics: Give broad-spectrum IV antibiotics (follow local trust guidelines).
  3. IV Fluids: Usually a bolus of crystalloid (e.g., 500ml of Hartmann's or Normal Saline).

qSOFA Score

While the full SOFA score is used in ICUs, qSOFA (quick SOFA) is a handy bedside tool to identify patients at high risk of poor outcomes outside the ICU. It’s super simple—you only need to remember three things (Score $\ge$ 2 is high risk):

  • Altered Mental Status: GCS < 15.
  • Systolic Blood Pressure: $\le$ 100 mmHg.
  • Respiratory Rate: $\ge$ 22 breaths/min.

Antibiotic Timing: The "Golden Hour"

The rule is simple but strict: IV antibiotics must be administered within 1 hour of the suspicion of sepsis. Research shows that for every hour of delay, the risk of mortality increases significantly.
