Pilonidal abscess
An infected pilonidal cyst (i.e, pilonidal abscess) with localized swelling and reddening is visible on the upper part of the natal cleft.
Stages and features of pilonidal disease
Asymptomatic: Pores may be visible in the intergluteal fold. However, as the patient is asymptomatic, this is an incidental finding.
Acute: Erythematous swelling, purulent discharge, pain, and general symptoms like fever indicate acute disease. Immediate surgical intervention is required.
Chronic: Recurrent discharge from the sinus tract, particularly when pressure is applied to the surrounding tissue. This form may become acute and therefore requires surgical intervention.
Pilonidal sinus resection
Natal cleft of a patient who has undergone resection of a pilonidal sinus
The surgical incision is elliptical and lateral to the natural crease of the natal cleft.
The surgical wound may be sutured or allowed to heal by secondary intention.
The aim of the procedure is to lateralize the scar (away from the natal cleft) to prevent recurrence.
